Service Coordinator - Mill Run Place

The Service Coordinator is responsible for coordinating programs and services to assist clients or residents in maintaining the highest practicable level of independence and physical, mental, and psychosocial well-being.

Requirements

• Bachelor's Degree in social work - preferred
• Two (2) or more years' experience working with social service delivery to elderly population
• Working knowledge of resources available in geographic location serving the community
• Membership and familiarization with American Association of Service Coordinators helpful
