This position is responsible for leading case management and the delivery of person-centred home and community care services to support clients to live in their homes for longer and stay connected to their local community.
Requirements
- Certificate IV in Aged Care or Diploma of Community Service
- Experience using client management systems
- Current Australian driver's licence
- Own vehicle
