Referral Services Specialist - Referral Mgmt - Full Time 8 Hour Days (Non-Exempt) (Non-Union)

The Referral Services Specialist provides care coordination services to promote quality outcomes by evaluating options and services required to meet an individual's health care needs. They work collaboratively with members of the Keck Referral Center team to assess needs, facilitate development of an interdisciplinary plan of care, and coordinate care across service sites.

Requirements

• Reviews all incoming referrals to initiate and maintain positive working relationship with patients and referring physicians
• Transcribes all incoming referrals into Ambulatory Referral Management
• Adheres to USC policies, procedures, and standards
