Job Description
Urban Resource Institute seeks a Client Care Coordinator (Social Worker) to work with homeless families in New York City, assessing mental and emotional functioning and providing clinical supervision to Case Managers.
Requirements
- Assess and evaluate existing, new and prospective clients/families
- Develops individual living plans (ILPs) for families
- Responsible for the clinical supervision of Case Managers
- Coordinates mental health referrals
- Attends trainings and consortium meetings
