Case Manager | CORE II

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Provides psychosocial rehabilitative services as described by the Medicaid manual, supports clients in following individualized care plans, and distributes medications as necessary.

Requirements

• High School diploma or equivalent
• None - see Preferred Qualifications
• Case Manager certification (within 90 days)
• CPR certification
• Valley de-escalation certification
• Driving positions require a minimum age of 21 and a current driver’s license
