Job Description
The Transitional Housing Case Manager provides comprehensive case management services to veterans participating in the Transitional Housing Program. This position supports veterans in achieving housing stability, increasing income, improving wellness, and transitioning to permanent housing.
Requirements
- AOD Registered and/or Certified, or BA/BS in related field, preferred
- Basic understanding of the military and the Veterans Administration
- Minimum of 6 months of paid case management experience in related field, preferred
- Prefer prior experience working with homeless population
- Knowledge of HMIS databases a plus!
