Job Description
The Housing Stabilization Case Manager is responsible for coordinating and delivering quality direct case management services to residents who have recently moved out of shelter into permanent housing.
Requirements
- Act in accordance with Victory Programs, Inc.'s Standards of Excellence & Philosophy of Care at all times
- Provide monthly case management services
- Transport and accompany clients to housing, legal, and medical appointments
- Develop and facilitate groups and workshops
- Maintain accurate documentation and record keeping
- Conduct outreach to current and potential landlords
- Work closely with other Case Managers
