Job Description
Franklin VNA & Hospice is seeking a full-time RN Case Manager to provide case management services to patients and families with complex needs. The position requires a caring and therapeutic environment with adherence to established best practices. The nurse will perform assessments, develop individualized care plans, and collaborate with interdisciplinary teams.
Requirements
- Graduate of an accredited nursing school
- Current certification/licensure as a Registered Nurse in New Hampshire
- 1+ year of progressive professional experience in a homecare setting
- Proficiency in assessment and communication skills
- Ability to work independently and collaboratively
- Strong ethical and interpersonal skills
