Job Description
Registered Nurse Home Health Case Manager: Care for patients in their homes, overseeing implementation and ongoing assessment of patient plans, promoting preventative care, and ensuring continuity of care.
Requirements
- Current license to practice as a Registered Nurse (RN)
- Current CPR certification
- Valid state-issued driver's license, automobile insurance, and reliable transportation
- One (1) year of professional experience practicing as a Registered Nurse (RN) in a home health or similar setting
