LPN Continuous Care

As an LPN with VITAS Healthcare, you will provide one-to-one crisis care for hospice patients in their homes or assisted living/skilled nursing facilities. You will work in shifts of 8-12 hours and collaborate with an interdisciplinary team to provide holistic support for patients and families.

Requirements

• At least one year of hospital nursing experience
• Reliable transportation
• Nursing license
