Nurse Practitioner

Nurse Practitioner will record physical findings, formulate plan and prognosis, discuss case with physician, and prescribe or recommend treatment. Must have 2 years experience in hospice, home health or community health and completion of accredited Nurse Practitioner program.

Requirements

• Minimum 2 years experience in hospice, home health or community health in the past 5 years
• Completion of accredited Nurse Practitioner program
• State licensure
