Job Description: Records physical findings, formulates plan and prognosis, and prescribes treatment for patients. Collaborates with health professionals to prepare comprehensive patient care plan. Monitors patient progress and reports to supervising physician.
Requirements
- 2+ years experience in hospice, home health, or community health
- completion of accredited Nurse Practitioner program
