Social Worker

VITAS Healthcare is a leading end-of-life care provider seeking a Social Worker to provide emotional and logistical support to patients and their families during end-of-life care. This role involves ensuring patients reach their wishes for finances, funeral arrangements, and offers comfort and dignity to patients. The position is diverse and involves both independent work and collaboration with interdisciplinary teams.

Requirements

• Ability to travel per job requirements.
• Thorough knowledge of community resources.
• Knowledge of psychosocial issues of terminally ill patients.
• Capacity to relate with others and work as part of an interdisciplinary team.
• Excellent listening and communication skills.
• Experience with terminally ill and families preferred
• Licenses and/or Certifications
• Bachelor’s degree required (LBSW preferred) from an accredited school.
• BSW (SW) optional.
• Master's degree may be considered if required by State Regulations
