Social Worker

VITAS Healthcare is seeking a compassionate and skilled Social Worker to provide emotional and logistical support to hospice patients and their families. The role involves ensuring patients reach their end of life with proper arrangements and offering support during the grieving process. Social workers will be responsible for communicating with families, managing patient needs, and advocating for patients within the healthcare system.

Requirements

• Ability to travel per job requirements.
• Thorough knowledge of community resources.
• Knowledge of psychosocial issues with terminally ill patients.
• Excellent listening and communication skills.
• Licensed Master Social Worker (LMSW) or equivalent.
• One year of social work experience in a health care setting.
• At least one year of experience as a social worker with terminally ill/family patients.
