Job Description
VITAS Healthcare is seeking a Hospice Social Worker to provide emotional and logistical support to patients and their loved ones during a vulnerable time. The role involves ensuring patients reach the end of life with necessary affairs and connecting them with available resources. Requirements
- Ability to travel per job requirements.
- Knowledge of community resources (or ability to obtain).
- Knowledge of psychosocial issues with terminally ill patients.
- Experience working with terminally ill and their families (preferred)
- Master's degree in Social Work
- At least one year of social work experience in a health care setting.
- License and/or Certification as required by State.
