This position is for a Social Worker within the interdisciplinary team, responsible for psychosocial assessment, counseling, and case management for patients and families. The role focuses on providing psychoeducation, referral support, and ensuring continuity of care. The role involves training, volunteer coordination, and potential spiritual care rotations.
Requirements
- Ability to travel.
- Knowledge of community resources.
- Knowledge of psychosocial issues in terminally ill patients.
- Excellent communication and interpersonal skills.
- Ability to advocate for patients and families.
- Masters of Social Work (LSW) license or equivalent (at least one year of experience) or MSW license and 1+ year in health care.
