Social Worker (SW)

They are seeking a Social Worker to provide emotional and logistical support, ensuring patients' wishes are honored, and offer bereavement support. The position involves working with both patients and families in various settings, including private residences and skilled nursing facilities.

Requirements

• Ability to travel per job requirements.
• Thorough knowledge of community resources.
• Knowledge of key psychosocial issues with terminally ill patients.
• Capacity to relate and work with a team.
• Excellent communication and listening skills.
• Experience as a social worker with terminally ill patients, preferred.
