We are seeking a Case Manager to join our healthcare team, providing comprehensive and compassionate case management services to clients. Your role involves collaborating with healthcare professionals and others as needed to address our clients’ medical, social, and emotional needs, ensuring they receive the highest quality of care.
Requirements
- Works scheduled hours within the clinic setting as part of the internal care team, serving all Vivent patients seen in our clinic.
- Conduct thorough assessments of clients' medical and psychosocial needs to develop individualized case management plans.
- Collaborate with healthcare providers to coordinate and facilitate access to medical services, ensuring a holistic approach to patient care.
- Perform eligibility assessments and collect eligibility proof documentation to facilitate access to grant funded services.
- Act as an advocate for clients within the healthcare system, ensuring their voices are heard and their needs are met.
- Maintain accurate and up-to-date documentation of case management activities, adhering to organizational and regulatory standards.
