Health Coach (Part-Time)

We are seeking a part-time Health Coach to empower individuals to make informed decisions about their health and well-being. The Health Coach will develop personalized health plans, provide guidance on lifestyle modifications, treatment adherence, and foster a supportive environment for clients to achieve their health goals.

Requirements

• Conduct comprehensive health assessments to understand clients' needs, goals, and challenges.
• Develop individualized health plans tailored to clients' unique needs and circumstances.
• Provide guidance on treatment adherence, nutrition, exercise, stress management, navigating systems of care, and other health-related areas.
• Build strong relationships with clients, fostering a supportive and motivating environment.
• Regularly communicate with clients to monitor progress, address concerns, and provide encouragement.
