Job Description
The Case Manager for the Mobile Medication Unit (MMU) provides direct client support, facilitates care coordination, and assists with discharge planning for individuals served by the MMU. They interact with community resources, promote client engagement, and ensure positive client outcomes. This role involves both in-home visits and travel to designated locations.
Requirements
- Conduct intake screenings and complete admission documentation.
- Develop individualized service plans and discharge plans.
- Linkage to medical, behavioral health, housing, and social support.
- Monitor client attendance, engagement, progress toward treatment goals, and adherence to MOUD.
- Maintain accurate, timely documentation in the EHR.
- Provide client advocacy and support.
- Assist clients with transportation and benefits enrollment.
