Job Description
The Targeted Case Manager will provide comprehensive case management services to patients with behavioral health needs, specifically related to induction of Buprenorphine (Suboxone).
Requirements
- Bachelor's degree in Human Services, Social Work, Psychology, Counseling, or related field
- Prior experience working with target populations
- Experience and knowledge of Microsoft systems and electronic health records (EHR)
- Excellent written and oral communication
- Ability to speak, read and write the English language
- Bilingual English/Spanish a plus
