Social Worker will work with a frail senior population, conducting social work assessments and delivering standard social work services. The role involves collaborating with an interdisciplinary team and establishing deep relationships with participants. The Social Worker will also help participants age safely and independently in their homes with community resourcing.
Requirements
- Master’s degree in social work (MSW) required
- Minimum of one (1) year of experience working with a frail or elderly population preferred
- Experience coordinating and facilitating care conference meetings preferred
- Experience assisting people with behavioral health or substance abuse issues preferred
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter
