Job Description
The Patient Care Advocate is a key member of the WellBe care team, providing compassionate care to frail elderly patients in their homes. They will develop a rapport with patients, deliver exceptional in-home services, and coordinate care with the medical team.
Requirements
- High school graduate or GED
- 2+ years of patient-facing experience in a healthcare practice or hospital
- Current BLS card for healthcare providers or willingness to obtain
- EMR documentation experience preferred
- Strong customer service and geriatric focus skills
