Job Description
The Patient Care Advocate is a key member of the WellBe care model, providing compassionate care to elderly patients with chronic and acute illnesses or injuries living in their homes. They will develop a rapport with patients, coordinate care with medical teams, and deliver exceptional in-home services.
Requirements
- High school graduate or GED
- 2+ years of patient facing experience working for a healthcare practice or hospital
- Current BLS card for healthcare providers or willing to obtain
- EMR documentation experience preferred
- Strong customer geriatric focus/service skills
- Strong computer skills, including Word, Excel, and PowerPoint
- Strong verbal, written, presentation, and interpersonal communication skills
