Patient Care Coordinator

The Patient Care Coordinator is part of a home-based care team that provides compassionate care to the frail population. The role is responsible for coordinating care, scheduling appointments, and providing customer service. The successful candidate will employ critical thinking and decision making, be comfortable with technology, and have flexibility.

Requirements

• High school graduate or GED
• 2+ years of experience working for a healthcare practice or hospital
• EMR documentation experience preferred
• MA Certification-Preferred
• Strong critical thinking skills
• Strong customer centric focus/service skills
• Strong computer skills, including Word, Excel, and PowerPoint
• Strong verbal, written, presentation, and interpersonal communication skills
