Job Description
Wellspring Agency Overview: Wellspring has championed mental health recovery in Louisville since 1982. The Assertive Community Treatment (ACT) team provides intensive services to individuals with serious mental illness, including medical, housing, and behavioral health support. The ACT Targeted Case Manager (TCM) assists clients in gaining access to services, developing recovery goals, and collaborating with community partners.
Requirements
- BA or BS in psychology, social work or related field
- KY TCM Training
- One year experience working with adults with serious mental illness
- One year experience working with adults with Substance Use Disorders (preferred)
