Targeted Case Manager (TCM) Forensic Assertive Community Treatment (FACT)

Wellspring is a mental health recovery agency that supports adults with psychiatric disabilities. They provide housing, employment services, and behavioral health services. They are seeking a Targeted Case Manager (TCM) Forensic Assertive Community Treatment (FACT) to assist clients with complex needs and provide intensive services. The ideal candidate will have a personal vehicle, a belief in recovery from mental illness, and experience working with adults with serious mental illness.

Requirements

• Assist clients in gaining access to medical, legal, social, educational and other support services.
• Develops and facilitates the pursuit of individualized recovery goals via assessment, connecting to resources and monitoring progress.
• Travels throughout the community conducting home visits and connecting clients with resources and services.
• Collaborates with landlords and other community partners, advocating for client needs and rights.
• Assists clients in locating, applying for, and moving into housing.
• Assists clients with illness management, medication management, and independent living skills, providing crisis intervention as needed.
• Participates and actively engages in weekly / monthly / quarterly staff meetings, on call rotations, and collaborates daily with Wellspring clinical and program staff as well as other providers and sources of support.
