Job Description
We are seeking an experienced Director of Social Services to lead and oversee the psychosocial care of residents within our clients facility.
Requirements
- Assess, identify, and document residents medically related social, emotional, and psychosocial needs
- Coordinate care with residents, families, interdisciplinary team members, and community resources
- Provide crisis intervention and emotional support to residents and families as needed
- Ensure accurate and timely documentation for MDS and Care Plans
