Job Description
We’re seeking a dedicated Advanced Practice Provider (NP/PA) to join our dynamic Direct Primary Care (DPC) team! As an Advanced Practice Provider for our agency, you’ll have the autonomy to provide full-spectrum primary care in a low-stress, high-impact environment.
Requirements
- Current Nurse Practitioner (NP) or Physician Assistant (PA) licensure
- Experience in primary care or family medicine preferred
- Passion for partnering with your patients and patient-centered care
- Ability to build trust and strong relationships with patients
- Excellent communication and clinical skills
- Independent, self-motivated, and proactive approach to care
- Ability to self-schedule and to prioritize patient needs
- Home health or hospice experience is encouraged
