Advance Practice Primary Care Provider - Part Time

We're seeking a dedicated Advanced Practice Provider (NP/PA) to join our dynamic Direct Primary Care (DPC) team. As an Advanced Practice Provider for our agency, you’ll have the autonomy to provide full-spectrum primary care in a low-stress, high-impact environment.

Requirements

• Current Nurse Practitioner (NP) or Physician Assistant (PA) licensure
• Experience in primary care or family medicine preferred
• Passion for partnering with your patients and patient-centered care
• Ability to build trust and strong relationships with patients
• Excellent communication and clinical skills
• Independent, self-motivated, and proactive approach to care
• Ability to self-schedule and to prioritize patient needs
• Home health or hospice experience is encouraged
