Job Description
The Community Health Worker serves as a vital link between healthcare providers and local communities, providing health education, connecting people with medical and social resources, and helping break down barriers to care. Dedicated to improving community health, the Community Health Worker establishes and maintains relationships between vulnerable populations and healthcare or social service systems, advocates on behalf of individuals, and participates in monthly Pathways Hub meetings.
Requirements
- Certified Community Health Worker, licensed through the Ohio Board of Nursing.
- High school diploma or GED required; associate or bachelor’s degree preferred.
- Valid Ohio driver’s license with access to a reliable, insured vehicle.
