The Western Reserve Area Agency on Aging (WRAAA) is a private nonprofit that serves as the planning, coordinating, and administrative hub for federal and state aging programs across Cuyahoga, Geauga, Lake, Lorain, and Medina counties. Partnering with local providers and the Ohio Department of Aging, WRAAA develops and implements long‑term services and supports that enable older adults to age successfully in their homes and communities. The agency’s distinctive focus includes seamless transitions between care settings, empowering seniors to manage their health, and specialized initiatives such as veteran dementia support in collaboration with the VA. WRAAA offers a range of services—from Medicaid enrollment and social work to advocacy and community resource coordination—making it a central provider of comprehensive aging support in the region.

Community Resource Consultant (RN/LSW)
Current licensure as a social worker or registered nurse, 2 years of experience in home care or geriatrics, and knowledge of medical terminology
LPN Patient Navigator
Current LPN licensure and 1+ year of experience working with individuals with physical or behavioral health disabilities required
CSSD Student Intern
Social work designee or BA student with excellent communication skills and basic computer proficiency
Assessor/ Care Manager
Licensure as social worker or RN, 1 year of experience, basic knowledge of chronic illnesses, disease processes, and medical terminology
MSW Social Work Intern - Aging Disability Resouce Center
Enrollment in a master’s degree program in Social Work (MSW), Licensed Social Work or Social Work Trainee (SWT) License required, Ability to travel in a multi-county service area
Social Work Case Manager
1+ year of experience, current licensure as a social worker, excellent interviewing and assessment skills
Social Work Intern - Aging Disability Resource Center
BSW intern; excellent interpersonal and verbal/written communication skills; ability to work effectively independently and as part of a team
Care Transitions Clinician
Active licensure, experience in care transitions, and ability to work independently
