Job Description
Join the Western Reserve Area Agency on Aging as a LPN Patient Navigator, where you will work as a liaison between Care Management staff, member and providers, and assist with completing Emergency Response Outreach to members during weather events or other emergency situations.
Requirements
- Current licensure as a License Practical Nurse with the Ohio Board of Nursing
- At least one year of experience working with individuals with physical or behavioral health disabilities in a home health care, medical, or behavioral health environment
- Valid Ohio driver’s license, proof of auto liability insurance
