Job Description

The role involves initiating the process of evaluating individual/consumer needs, determining eligibility for programs administered by WRAAA, and providing information regarding available options.
Requirements
- Current licensure as a Licensed Practical Nurse with the Ohio Board of Nursing
- One year of experience in home care, geriatrics, home health care, behavioral health, long term care, or other substitute as approved by the Ohio Department of Aging
- Knowledge of major chronic illnesses, disease processes, normal aging, medical terminology
- Excellent verbal and written interviewing and assessment skills
- Knowledge of local resources for older adults
- Excellent interpersonal and communication skills
- Intermediate computer skills including Microsoft Office applications
