Social Work Case Manager

Join Western Reserve Area Agency on Aging as a MyCare Ohio Case Manager and be part of an organization dedicated to providing choices for people to live independently. The position involves assessing and care managing long-term clients aged 60 and older enrolled in waiver programs.

Requirements

• Current licensure as a social worker by the State of Ohio Counselor and Social Work Board.
• At least one year of experience working with individuals with physical or behavioral disabilities in a home health care, medical social work, or behavioral health environment.
• Excellent verbal and written interviewing and assessment skills.
• Basic knowledge of major chronic disabling illnesses for persons of all ages, disease processes, and normal aging or special knowledge in pediatrics and young adults.
