Job Description
The Transitional Case Manager provides ongoing support to the criminal offender population both pre- and post release from incarceration. The role involves ensuring ongoing insurance authorizations, helping clients obtain suitable housing, transportation, identifying jobs skills, and other social needs.
Requirements
- Bachelor's degree
- Knowledge of the criminal offender population and community resources
- Good interpersonal and customer service skills
- Valid CT state driver’s license
