Case Coordinator

Case Coordinator at Southern Health-Santé Sud in Steinbach, Manitoba, Canada. • Graduate of an accredited post-secondary education program (B.N. or Social Sciences B.S.W., OT, PT) OR Registered Nurse with relevant experience in Medicine, Community, or Long-Term Care
• Active license and registration to practice as required by professional association
• Gerontological Nurse Certification (C) an asset
