Hospital Based Case Coordinator (N3) - Home Care

The Hospital Based Case Coordinator (HBCC) role involves receiving referrals, conducting assessments, and developing plans of care to support discharge from an acute care setting. The role also involves liaising with local health care services, hospitals, and community partners to determine the best care environment for clients. The successful candidate will have a minimum of three years recent related nursing experience and a graduate degree in an approved Registered Nursing education program.

Requirements

• Minimum three years recent related nursing experience
• Graduate of an approved Registered Nursing education program
• Certification in Procura software and assessment tool interRAI-HC preferred
• Active participation in professional associations e.g. Association of Regulated Nurses of Manitoba (ARNM) preferred
