Job Description

The Hospital Based Case Coordinator (N3) receives referrals, conducts assessments, and develops plans of care to support discharge from an acute care setting. In collaboration with clients, families, and interdisciplinary teams, the HBCC determines eligibility for Home Care program supports and coordinates delivery of a broad range of professional and non-professional services.
Requirements
- Graduate of an approved Registered Nursing education program required
- Minimum three years recent related nursing experience required
- Wound Care & Diabetes courses is an asset
- Canadian Community Health Nurses Certification CCHN or relevant applicable certification an asset
Benefits
- Pensionable Salary Enhancement - Nurses in eligible classifications may be eligible to earn an additional $5.95/hour when working full-time hours within a prescribed six-month reporting period
