Job Description
The Intensive Family Coordinator will provide case management services, direct advocacy, and individualized service plans to families. The role involves travel, working nights and weekends, and providing 24/7 crisis support.
Requirements
- High School Diploma or GED Equivalent
- 1 year experience working in human services fields
- Experience in home and community based services
- Proficiency in MS Office
- Knowledge of Electronic Health Record (EHR) systems
- Bilingual/Spanish speaking
- Reliable transportation, valid driver's license, and current auto insurance coverage
