Job Description
The Community Housing Case Manager - Part Time will deliver a trauma-informed response affirming the dignity of clients and facilitating a path of resiliency and recovery for individuals whose lives have been impacted by homelessness.
Requirements
- Develop individualized housing services that encompasses a housing service plan for each individual and/or family.
- Schedule referral appointments and complete the intake process with the individual and/or family in a timely manner.
- Assist individual or family’s utilizing the housing first model which includes trauma-informed care practices that will assist individual or families in securing housing.
