Job Description
ADAPT Community Network, formerly United Cerebral Palsy (UCP) of NYC, is seeking a Habilitation Assistant to support individuals in achieving greater independence and personal growth through daily activities and individualized programs. The role involves providing direct personal care, implementing skill development, promoting inclusion, and maintaining a supportive environment. This position requires adherence to stringent safety and well-being standards for individuals with disabilities.
Requirements
- Provide direct support and personal care to individuals.
- Support implementation of individualized program plans.
- Supervise individuals in classroom activities and community outings.
- Maintain close supervision of individuals during program activities.
- Collect data and document progress.
- Read, write, speak, and understand English.
- Respond to crisis situations.
