Patient Care Coach

This Patient Care Coach position supports Durable Medical Equipment (DME) product lines, focusing on patient adherence and device utilization. The role involves personalized education, data monitoring, documentation, and collaboration with internal and external teams to ensure a smooth and patient-centered experience. This position is an integral part of the care team and requires strong communication, technical aptitude, and a patient-centered approach.

Requirements
