Job Description
AIM is seeking a Health Home Care Manager to join our mission-driven organization, providing professional, consultative, investigative, advisory, and educational activities for patients and families, site staff, and collaborative community agencies.
Requirements
- Home and community visits
- Complete a comprehensive assessment of medical, behavioral health, and social service needs
- Provide education and information regarding community resources
- Collaborate with various community providers and resources
- Escalate care management when a medical assessment is needed
- Document plan of care, patient utilization, activities, and other required information
- Frequent non-medical management coaching, education, follow-up visits, and phone calls to patients
