The Case Manager position is responsible for coordination and continuity of care, providing life skills training and development in individual and group settings, and connecting clients to community resources.
Requirements
- Assists clients with adjusting to residential program and preventing unplanned discharges
- Coordinates care with stakeholders and family/natural supports for clients in residential program
- Provides individualized case management to residential clients addressing Social Determinants of Health
- Develops, implements, and reviews clinical documentation of all services provided through client records
- Collaborates with each assigned client on individualized service plan goals
- Assists in coordination of appropriate level of care for each assigned client
- Actively participates in interdisciplinary treatment team meetings
- Attends clinical oversight under Behavioral Health Professional, Clinical Supervisor or Clinical Manager
- Completes accurate clinical documentation same day as date of service
- Transports clients as needed
- Assists in the collection of client breathalyzer readings and/or urine drug screening
- Secures client information per HIPAA standards
- Attends and/or provides in-service training as needed
- Promotes mission, vision, and values of the Axiom Care program
- Promotes inclusion through Culturally and Linguistically Appropriate Services (CLAS) Standards
