Job Description
Axiom Care is seeking a Case Manager for its Detox program in Apache Junction, AZ. The Case Manager will work with clients to develop individualized treatment plans, assist with housing and employment services, and provide high-quality documentation of services provided.
Requirements
- Proficient in computer applications and electronic medical record systems
- Ability to communicate effectively with clients, client supports and professionals
- Ability to make sound decisions in emergency situations
- Working knowledge of twelve step programs of recovery
