NOVA - Case Manager II

As a Case Manager II at NOVA, your role will involve providing comprehensive support services to individuals who were incarcerated and being released. You will assess participants' needs, develop individualized service plans, and connect them with appropriate resources to help them regain stability and move towards permanent housing. Your primary objective will be to empower clients to overcome barriers and achieve self-sufficiency.

Requirements

• Conduct comprehensive assessments of individuals experiencing homelessness to identify their needs, strengths, and goals
• Develop individualized case plans in collaboration with clients, addressing their immediate and long-term needs
• Connect clients with appropriate resources and services, such as housing programs, mental health services, substance abuse treatment, job training, and legal assistance
• Advocate for clients' rights and needs within the shelter and in the community, including liaising with social service agencies, government entities, and community organizations
• Provide immediate support and intervention during crisis situations, such as mental health crises, substance abuse relapses, domestic violence incidents, or medical emergencies
• Regularly monitor clients' progress, document interactions, and maintain accurate and confidential case records
