SISTERS - Case Manager II

The SISTERS - Case Manager II role at Bayview Hunters Point Foundation involves providing comprehensive support services to individuals in a jail program, assessing needs, developing individualized service plans, and connecting clients with resources to regain stability and achieve self-sufficiency.

Requirements

• Conduct comprehensive assessments of individuals experiencing homelessness to identify their needs, strengths, and goals.
• Develop individualized case plans in collaboration with clients, addressing their immediate and long-term needs.
• Connect clients with appropriate resources and services, such as housing programs, mental health services, substance abuse treatment, job training, and legal assistance.
• Advocate for clients' rights and needs within the shelter and in the community, including liaising with social service agencies, government entities, and community organizations.
• Provide immediate support and intervention during crisis situations, such as mental health crises, substance abuse relapses, domestic violence incidents, or medical emergencies.
• Regularly monitor clients' progress, document interactions, and maintain accurate and confidential case records.
• Offer emotional support, guidance, and counseling to clients, helping them develop coping skills, problem-solving abilities, and positive relationships.
• Facilitate workshops and group sessions on life skills, including budgeting, job readiness, communication skills, and conflict resolution.
• Collaborate with other case managers, shelter staff, and external service providers to ensure coordinated and holistic care for clients.
• Assist in evaluating the effectiveness of the shelter's programs and services, providing feedback and recommendations for improvement.
• Stay updated on best practices in case management, homelessness, and related fields through ongoing training and professional development opportunities.
