Job Description
We are seeking a Case Manager, Transitional Housing to provide case management services to people experiencing homelessness in Brooklyn, NY. The successful candidate will have a bachelor's degree and 2 years of experience working with homeless adults and substance abuse issues.
Requirements
- Provide direct services to a caseload of approximately 15 clients
- Conduct psychosocial evaluations
- Connect clients to psychiatric services, medical treatment, and substance use treatment facilities
- Obtain identification, public benefits, and other applicable income sources for clients
- Maintain record keeping in compliance with agency standards and City and State agencies
- Intervene in crisis situations
