Temporary Community Health Worker

The Community Health Worker (CHW) role at Brighter Beginnings supports the implementation and oversight of grant-funded activities through program monitoring, documentation, data collection, quality assurance efforts, and assistance with program reporting. The CHW connects patients with health, social, and supportive resources while helping to reduce barriers to care. Responsibilities include providing sexual and reproductive health education, outreach, resource navigation, patient scheduling, appointment follow-up, and documentation within the electronic health record (EHR).

Requirements

• Lived experience that fosters a meaningful connection to the population served
• Experience in community education, outreach, health promotion, resource navigation, social service linkage, or advocacy for community health and social service needs
• Excellent interpersonal, communication, and relationship-building skills
• Strong organizational, time management, and computer skills
• Ability to work independently and within a multidisciplinary team environment
• Possession of a valid California Driver's License, current automobile insurance, and access to a reliable personal vehicle
• Bilingual proficiency in English and Spanish or another language commonly spoken within the communities served
• Associate's degree or higher in Public Health, Sociology, Nursing, Social Work, Health Education, or a related field
